What is the speed of the carving (mm/sec or mm/min). Over driven. It looks like you're losing steps on the Y axis. A power surge could have mucked up some of the settings.

I agree with gerry - while there is a chance for a mechanical issue, I would get the root files from Axiom and re-initialize the controller to base settings and just make sure it's not something as simple as that. Also look for any changes that might have happened with electrical grounding that might lead to electrical noise. Sometimes even modifications to the dust collection system can induce new static electrical charges that can cause some loss of data.
Also check (and lubricate) the the linear slides along the sides that the Y-axis travels on.

The feed rate was at 100 in/min. I had the kids back that down to 80 in/min. We have never had to mess with this setting in the past but thought that is where I should start. Still an issue at 80 in/min.
